Acute stent malfunction in a calcified lesion

چکیده
Stent fracture is an emerging complication of coronary stenting. Stent fractures were originally reported in the superficial femoral arteries but have since been reported in almost all vascular sites. They are the result of the complex interplay among stent design, the stented segment, plaque morphology, deployment technique, including forceful exaggerated motion in the atrioventricular groove as observed in right coronary artery, long stent, tortuous lesion, stent overexpansion, complex lesion after stenting of a totally occluded vessel, and calcified lesions. Their presentation is highly variable, ranging from asymptomatic, perforation, restenosis, migration of the stent to sudden cardiac death related to stent thrombosis. They have been reported as acute consequences as well as delayed presentation. Stent fractures were initially reported with bare metal stents but are now seen with drug-eluting stents as well. Herein, a case of stent fracture of a third-generation zotarolimus-eluting stent in a 60-year-old woman is reported, with a moderately calcified lesion that was diagnosed by stent boost and successfully treated with restenting using another drug-eluting stent.

Managing a complication after direct stenting: removal of a maldeployed stent with rotational atherectomy.
A 40 year old patient presented with acute anterior wall infarction. A non-calcified lesion was stented directly without significant expansion of the stent. Rotational atherectomy successfully removed parts of the maldeployed stent and resistant arterial wall substance allowing full dilatation of the lesion.
متن کاملSTATE-OF-THE-ART PAPER Selection of Coronary Stents
In clinical practice, the operator must decide which stent is most appropriate for the patient. This article focuses on the features of stent design that make a specific stent more or less suitable for a particular type of lesion or anatomy: the “average” coronary lesion, the lesion situated on a curve, the ostial lesion, the bifurcational lesion, the lesion located at the left main stem, the c...
متن کاملThin-strut drug-eluting stents are more favorable for severe calcified lesions after rotational atherectomy than thick-strut drug-eluting stents.
AIM Percutaneous coronary intervention (PCI) for severe calcified lesions is still challenging, and there are few studies of drug-eluting stent (DES) implantation for severe calcified lesions, especially regarding long-term results and hemodialysis patients. The study purpose was to clarify the factors, including DES strut thickness, that affect the long-term outcome of severe calcified lesion ...
متن کاملTwo-Year Clinical Outcomes of Newer-Generation Drug-Eluting Stent Implantation Following Rotational Atherectomy for Heavily Calcified Lesions.
BACKGROUND Clinical outcomes of implantation of the newer-generation drug-eluting stent (DES) following rotational atherectomy for heavily calcified lesions remain unclear in the real-world setting. METHODSANDRESULTS We enrolled 252 consecutive patients (273 lesions) treated with newer-generation DES following rotational atherectomy. The primary endpoint was the cumulative 2-year incidence of...
متن کاملCutting-balloon angioplasty before drug-eluting stent implantation for the treatment of severely calcified coronary lesions
BACKGROUND Severely calcified coronary lesions respond poorly to balloon angioplasty, resulting in incomplete and asymmetrical stent expansion. Therefore, adequate plaque modification prior to drug-eluting stent (DES) implantation is the key for calcified lesion treatment. This study was to evaluate the safety and efficacy of cutting balloon angioplasty for severely calcified coronary lesions. ...
